OUR APPROACH
Graceful Aging Home Care knows that the symptoms of Alzheimer’s and dementia can vary widely depending on the type of the dementia, the stage and more importantly the individual. No one care plan fits all. Our care manager will work with each individual family to come up with a care plan that is right for your loved one and will adjust the care plan as the disease progresses.
Our initial focus is the safety and security of the client by preventing wandering and falls. Our long term goal is building a cooperative relationship with the client. The caregiver will get to know the client and come up with strategies to limit confusion and promote trust. Once that is achieved our caregivers can focus more on managing the client’s activities of daily living and interests leading to a greater quality of life.
Stroke Care
The unexpectedness of a Stroke can be both physically and emotionally traumatizing. The realization that you can no longer live at home without help can be hard to cope with. Recovery from a stroke can be long and daunting for you and your family. The skilled caregivers at Graceful Aging Home Care who specialize in stroke care can help with your recovery whether you recently had a stroke or living with the side effects of a previous stroke.
OUR APPROACH
Our care manger will meet with you and your family to fully understand your symptoms and how they are impacting your daily routine along with your goals and objectives. It is also imperative to understand the treatment plan developed by your healthcare specialists with the goal of developing a home care plan that accompanies and supports your continued rehabilitation. Once your individualized care plan is created we match you with a caregiver best suited to meet your needs. Once matched, there is intensive client specific training to ensure your caregiver understands your customized care plan and how to support your ongoing recovery. Periodic check-ins with your care manager will assure your care plan gets updated as you make progress. Our stroke care services include:
Companion Care
Depression affects more than 19 million Americans regardless of age, race or gender. While depression is not a normal part of the aging process, it is more likely to occur in older adults experiencing other physical health conditions. For example, almost a quarter of those who will experience a stork in a given year will also experience clinical depression. Unfortunately, symptoms of depression are often overlooked and untreated when they coincide with other medical illnesses or life events that commonly occur in older adults (i.e. the death of a spouse).

Graceful Aging Home Care’s companion care services focus on improving your emotional well being by helping you cope with feelings of depression and loneliness so you can continue to age gracefully. We get to know our clients unique histories and personalities so we can plan activities that are both stimulating and enjoyable for them inside and outside the home.
