Our care manager will meet with you and your family to fully understand your symptoms and how they are impacting your daily routine along with your goals and objectives. It is also imperative to understand the treatment plan developed by your healthcare specialists with the goal of developing a home care plan that accompanies and supports your continued rehabilitation. Once your individualized care plan is created, we match you with a caregiver best suited to meet your needs. Once matched, there is intensive client specific training to ensure your caregiver understands your customized care plan and how to support your ongoing recovery. Periodic check-ins with your care manager will assure your care plan gets updated as you make progress.
