OUR APPROACH

Graceful Aging Home Care knows that the symptoms of Alzheimer’s and dementia can vary widely depending on the type of dementia, the stage and more importantly the individual. No one care plan fits all. Our care manager will work with each individual family to come up with a care plan that is right for your loved one and will adjust the care plan as the disease progresses.

 

Our initial focus is the safety and security of the client by preventing wandering and falls. Our long-term goal is building a cooperative relationship with the client. The caregiver will get to know the client and come up with strategies to limit confusion and promote trust. Once that is achieved our caregivers can focus more on managing the client’s activities of daily living and interests leading to a greater quality of life.
